3. Guide of the Information Regulator

The Information Regulator of South Africa has compiled a guide in terms of Section 10 of PAIA containing information to assist persons wishing to exercise any right contemplated in PAIA and POPIA. The guide is available from the Information Regulator:

4. Records Automatically Available Without Request

The following categories of information are automatically available on our website without submitting a formal PAIA request:

• Service descriptions (Rife Wave Therapy, Bioresonance Scanning, Quantum QMRA Analysis).
• Practitioner credentials and SATAHWA registration details.
• Transparent fee structures and ZAR consultation rates.
• Public educational articles, wellness guides, and health blogs.

5. Categories of Records Held by Rife Wellness Centre

The following records are maintained by Rife Wellness Centre and may be requested subject to PAIA grounds of refusal:

• Client & Patient Records: Consultation intakes, bioresonance frequency scans, health history intake questionnaires (processed under Section 32 of POPIA with explicit consent).
• Operational Records: Service logs, appointment scheduling records, website analytical data.
• Financial Records: Invoices, payment records, tax documents, accounting records.
• Administrative Records: Statutory filings, supplier contracts, internal policies.

6. Request Procedure under PAIA

1. The requester must complete Form 2 (Prescribed PAIA Request Form) available from the Information Regulator.
2. Submit the completed form to the Information Officer via email at bryan@rifewellnesscentre.co.za.
3. Provide sufficient detail on the request form to enable the Information Officer to identify the record and the requester.
4. The Information Officer will notify the requester of the decision within 30 days of receipt of the request.

7. Protection of Personal Information Act (POPIA) Compliance

Rife Wellness Centre processes personal and health information strictly in accordance with POPIA conditions:

• Purpose Specification: Data is collected solely to schedule appointments, conduct wellness assessments, and deliver non-invasive frequency therapy sessions.
• Data Security Safeguards: Digital health records are encrypted on secure servers, and physical records are stored in restricted-access areas.
